Before completing this application please ensure you have read the Licensing & Regulatory Committee’s Sexual Entertainment Venue Policy

All mandatory questions must be answered. If these questions are not answered, the application will be deemed incomplete and will not be processed by the Licensing Section.

5. Please note you are required to upload a copy of the current SEV licence when submitting this application for renewal. You will also be required to submit:
  • Floor Plan for the premises;
  • Code of Conduct for performers within the premises; and
  • The premises written policies and procedures
1. A variation should be applied for where you wish to make a change to the terms and conditions of the SEV Licence
2. Please state the nature of the variation (for example change of layout/change of operating times/change to licence condition)
3. Please provide a description of the variation
4. If your variation relates to a change of layout for the licensed SEV premises please upload an amended layout plan showing the variation on the next page
